Enter the code from the table to correspond with the Question type (Rating 1-5, Yes/No, or enter one character such as "S" for Speaker or "M" for Material.

NOTE:  The list of evaluation questions must be created in the Lookup Codes under Meetings/Event Evaluation Questions.
